New patient guide
Everything first-time patients ask us, in one place.
What should I bring to the first visit?
Photo ID, your insurance card, a list of medications and supplements you take (or the bottles), and any recent test results. If you are transferring from another practice, we send the records request for you.
How long does the first visit take?
Plan 45 minutes. The first 10 are with a nurse for vitals and history, the rest with your physician. Follow-ups are usually 20 minutes.
Can I fill in forms before I arrive?
Yes. Once we confirm your appointment we email a link to the new patient forms. They take about 10 minutes and save you time at the desk.
Which insurance do you accept?
Most major plans and Medicare. Our front desk checks your coverage before the visit and tells you about any copay. Self-pay rates are on request.
Can my whole family see the same physician?
Yes, and many families do. Every physician here sees newborns to grandparents. You can also choose a different physician for each family member.
What if I need care after hours?
Call the main number; the recording gives the on-call physician's line for urgent questions. In an emergency, call your local emergency number.
